Please turn in the following to secure your roster spot as teams are filling fast!!!!!

  1. Fill out a Registration form (Click Here)
  2. Fill out a Medical Authorization Form  (click here)
  3. Read and Sign Code of Conduct Form  (click here)
  4. Copy of Birth Certificate
  5. Send it ALL into:  P.O. Box 203   Mukilteo,Wa 98275

All registrations must be PAID IN FULL and all paperwork turned in prior to gear suit ups.
